On Monday September 23^rd, students will receive their data verification information and student insurance forms in their homeroom class.  Students with a first period spare can pick up their forms from the library.

Please review the Data Verification Form (DVF) carefully, make any necessary changes, sign and date the bottom of the 2^nd page.  This form is required to be returned by each student attending in class programs at Eastwood Collegiate.

All student DVF forms must be returned to their homeroom teachers no later than Wednesday September 25^th.

If your student has medical concerns, please ensure this information is added on the first page of the DVF form.

High School students are not insured while on school property, please review the insurance forms and if interested follow the information provided in the envelope.
